The following are available upon request:
- •Policies and practices, including general eligibility criteria and fees;
- •A sample written adoption services contract substantially like the one that the prospective client(s) will be expected to sign should they proceed;
- •Concerns about the safety or quality of care provided by Adoption Assistance may be reported to The Joint Commission. If staff reports such concerns, no disciplinary or punitive action will be taken.
